The YMCA Healthy Living Center (Y-HLC) is a partnership between the YMCA of Greater Des Moines and Mercy Medical Center. The Y-HLC also partners with many other health organizations on a referral and programming basis as we together, profoundly address the needs of our entire community.
Located in Clive, IA, the Y-HLC is a community health center by means of enriching the patient experience, improving the health of populations, and partnering with healthcare organizations to reduce the cost of healthcare. We reach into the everyday lives of individuals and partner with them to influence healthy behaviors and self-efficacy.
This clinic to community initiative has begun a one-of-a-kind approach to health care in central Iowa. The Y-HLC brings together medical and fitness professionals to help individuals achieve the best possible health care and overall quality of life. This approach represents a focus on prevention and lifestyle changes as an essential part of health care. The Y-HLC incorporates the Y mission of building healthy spirit, mind, and body for all with services that relate to the whole person.
Health care professionals write referrals for their patients to the Y-HLC medical programs to help them address risk factors, manage chronic disease, improve symptoms, and more. From strategic health planning to coordinated care management and personalized fitness plans, the Y-HLC provides services for almost everyone. Whether an individual is undergoing treatment or recovering from an illness or injury, or is striving to improve his or her sports performance, the Y-HLC staff and medical partners are here to provide assistance and support through a wide array of programs and services.
